- Only keep your appointment if you are symptom free. If you are symptomatic, please cancel your appointment or change your appointment to telehealth.
- Please wait in your car or outside until your clinician or office manager indicates that they are ready.
- You need to wear a mask upon entry to the office, in all common areas, and throughout your time here. All staff and clinicians will adhere to this as well.
- Use alcohol-based hand sanitizer when you enter the building.
- Your temperature will be taken upon arrival. If it is elevated (100 degrees Fahrenheit or more), you will be asked to reschedule your appointment.
- Adhere to the social-distancing precautions upon entering the office and maintain a distance of 6 feet between you and others. There will be no physical contact (e.g., no shaking hands) with staff, clinicians, or other patients.
- Please refrain from unnecessary contact (e.g., please do not move chairs or sit where we have taped off furniture).
- No children or other companions are allowed at your appointment.
- If you use the restroom, please sanitize the toilet, sink, and doorknob before exiting.
